Fit Notes

Please be advised that to request a sick note, you will need to schedule a face-to-face appointment with your GP. To book your appointment, please call 01213274427 after 11:30 am.

When is a sick note required?

For absences of 7 days or less: If you are off work sick for 7 days or less, your employer should not require medical evidence of your illness. Instead, they may ask you to confirm your illness by filling out a form upon your return to work. This process is known as self-certification.

For absences of more than 7 days: If you are off work sick for more than 7 days, your employer will likely request a fit note (also known as a Statement of Fitness for Work) from a GP or hospital doctor. Fit notes may also be called medical statements or doctor’s notes.
